Server 2008 R2 Software RAID in failed status. Only one drive has failed.

Hi,

I have a server running Server 2008 R2.  I created a six (6) disk software RAID 5 array within Disk Management.  I had one drive fail due to a SMART failure.  I replaced the drive physically and was researching the steps to add the disk into the array and reactivate the disk.

While researching (between work and other things this took a couple days) the array went into a Failed state.  I have checked each disk for problems and they all show up properly in multiple systems.  The array however still shows as failed no matter what system they are in.  I am able to bring the 5 remaining drives "Online" in Disk Management, but when I "Reactivate Disk" I get the error: "The attempted operation cannot be completed. The selected volume is offline."

One thing I did note on the last attempt is when I had all 5 drives hooked up and went into disk management, they were listed as foreign disks, as expected, but when I imported the foreign disks, they showed up in three different groups.  A 1 disk group, a second 1 disk group and the remainder in a third group.

Disk management shows all 5 drives online in a Failed state with the sixth drive showing as "Missing".

Any ideas on how to recover this array?

Thanks in advance.
